If you run a franchise consulting or franchise development process, you likely already have a Confidential Questionnaire (CQ), sometimes called a Candidate Questionnaire, in some form - a PDF, a Google Form, a DocuSign packet, or a page on your current website. Here we'll walk through mapping your CQ into a VipeCloud Sign Up Form, and point you to the setup articles you will need along the way.
If you are starting a CQ from scratch and want a reference point, common questions include net worth, liquid capital, investment timeline, preferred territory, and financing plan. Everything below applies whether you are migrating an existing CQ or building one for the first time.
Step 1: Check What's Already in Your Contact Fields
Go through your existing CQ question by question. If you have an FSE account, most of what a CQ asks - net worth, liquid capital, timeline, territory - is probably already set up as a Contact Field. This step is less about building fields from scratch and more about spot-checking what already exists against the exact questions your CQ asks.
Navigate to your Contact Fields page. Standard information like name, email, phone, and job title lives under Standard Fields. The franchise-specific qualification fields live under Custom Fields, and an FSE account typically comes with a full set of these preloaded. Modify any preloaded field that doesn't quite match your wording, and create new ones to fill any gaps your CQ has that aren't already covered.

Step 2: Build the Form
Create the form as a Lead Capture form, since the candidate fills this out themselves. Full setup steps are in How To Create a Sign Up Form.
Note that when creating your form, you will want to add your fields as "Contact Field" rather than "Field for Form" in nearly all cases. In short - the reason for this is so the information that's captured will be added to the Contact record. More on this here.
Step 3: Carry Over Your Existing Wording
As you add each Contact Field to the form, you will likely want to relabel it so the information being requested is displayed in a candidate-friendly manner.
As an example, your "Net Worth" field can be presented as "What is your net worth?" in your CQ.
The relabel option is called Edit Display, covered in Sign Up Form Fields & Headers.
Step 4: Structure Your CQ
Add section headers so that your CQ is easily digestible for your candidates. And add page breaks so candidates aren't intimidated by how many questions there might be.
Step 5: Design Your Landing Page
Make your page fit your brand by adding a banner image, a header, and a description.
Step 6: Automate What You Already Do By Hand
Think about what happens today when a CQ comes in - a confirmation email or text, a stop to any reminder messages, a task for someone to review it - and rebuild that as an automation instead of doing it manually going forward.

Step 7: Get Your CQ Link And Send It To Candidates
You can locate your CQ link in two places:
From your Sign Up Forms table:
Go to Automations>Sign Up Forms and locate your CQ
Hover over its row and click "Preview"
That will open a new tab with your CQ loaded. Copy that URL and share it wherever needed.
From your Sign Up Form editor:
If you are actively editing your CQ Sign Up Form, you can locate your CQ Landing Page Link in the upper left of the editor.
Note that this link is a short link, which is ideal for texting.
These links are what you'll use to add your CQ to email and text templates, or to send to people in ad hoc communications. As you make ongoing edits and refinements to your CQ, the links won't change.
Other Names for a CQ
This process goes by different names depending on the brand, consultancy, or region you work with. If your team calls it something other than a CQ, this guide still applies. Common alternate names include:
