How To Create a Sign Up Form

Sign up forms let you collect contact information, trigger automations, and route new leads or contacts into the right place in your CRM. You can share them as a standalone landing page or embed them directly into your website.

How to Create a Sign Up Form

  1. Click Automation in your left sidebar.

  2. Under Lead Capture, click Sign Up Forms.

  3. Click Create.

  4. Name your form and select a Form Type.

  5. Click Create to open the form editor.

Form type options:

  • Lead Capture - Collects contact information and can trigger automations. Use this for contact forms, event registrations, and general lead capture. When in doubt, choose this one.

  • Sale / Donation - Works like Lead Capture but collects payment through PayPal at the final step.

  • Category Quiz - Presents questions that sort respondents into Category A, B, C, or D based on their answers.

How to Add Fields to Your Form

Fields determine what information your form collects. Use the dropdowns inside the form editor to add each type.

Field types available:

  • Contact Fields - Standard and custom fields that map directly to a contact record. Includes First Name, Last Name, Email, Phone, and any custom fields in your account. Submitted data is saved to the contact's record or updates an existing one.

  • Create Field for Form - A one-off field specific to this form only. Data collected here does not become a system-wide custom field. Useful when you are a subuser without custom field access, or when you only need the data for a single use case.

  • Consent Fields - Checkbox fields for Privacy Policy, Terms of Service, or Texting Opt-In. Use when explicit consent is required.

  • Section Headers - Visual dividers to organize your form when collecting several pieces of information.

How to Configure Form Details and Styling

The Form Details tab controls how your form looks and behaves for the person filling it out. It is divided into three subsections.

Button, Theme, and Captcha controls the submit button text, button style, and color theme. Use Dark Mode if you are embedding the form on a website with a dark color scheme. Enable Captcha if you want to reduce spam submissions - if you are using an embedded form, increase your iframe height to accommodate it.

Limit Sign Ups lets you cap the total number of submissions this form will accept. This is useful for event registrations, beta programs, or any situation where you have a fixed number of spots available. When the limit is reached, you can either display a custom message to visitors or redirect them to a different page.

What Does the Visitor See After Sign Up? controls the thank you message displayed after a successful submission. You can customize the message text or redirect visitors to a different URL instead - commonly used for conversion tracking on a thank you page.

How to Share Your Form

There are two ways to share a sign up form.

Landing Page Link

  • A shareable URL that opens a full standalone page.

  • Displays the banner image, page title, and description alongside the form.

  • Use this if you do not have a website, or if you want a quick standalone page.

Embedded Form

  • An iframe embed code you place directly into your website.

  • Displays only the form fields - no banner image or page description.

  • Ideal for contact pages, event pages, or other website-native experiences.

How to Customize the Landing Page

If you are sharing your form as a landing page, you can customize how it appears to visitors.

The Landing Page tab controls how your form appears when shared as a standalone page. You can set a page title, add a banner image and optional background image, and write a page description that supports text, images, and video. Use the preview button to see your changes in real time.

How to Set Up Automations

The Automations tab lets you trigger actions automatically when someone submits the form - such as sending a confirmation email, scheduling a follow-up message, or starting a series.

How to Configure Admin Settings

Admin Settings controls what happens internally after someone submits the form.

  • Use Assign Signups to route new contacts to a specific user or team.

  • Add Completion Alerts to notify teammates or contacts when the form is submitted.

  • Use Add Signups to a Contact List to automatically place new contacts into a list - for example, a newsletter list.

How to View Signups

  • Click the Signups tab inside the form editor.

  • View all contacts who have submitted this form.

  • Click any entry to open or edit that contact's record.

  • Click Send Email to send a message to everyone who filled out the form.

How to View Form Analytics

  1. Click the Analytics tab inside the form editor.

  2. Review total form views, total completions, and trends over time.

Analytics are useful for testing layout changes or call to action variations to see what improves completion rates.

Key Notes

  • Custom fields must exist in your account before they can be added to a form as Contact Fields. If a field is missing, create it first under account settings.

  • Subusers cannot create account-wide custom fields. Use Create Field for Form if you need to collect one-off data without admin access.

  • The embedded version of a form does not display the landing page content - only the fields and submit button are shown.

  • If you enable Captcha on an embedded form, increase your iframe height to accommodate the extra element.

  • Redirecting users after submission is commonly used for conversion tracking pixels on a thank you page.

  • Page breaks work on both the landing page and embedded versions of a form. If your form uses page breaks and is embedded, make sure your iframe height is tall enough to accommodate each page.
